LAS Alternate Format Text Request Form

Please read before completing:
Please use this form to request course materials - textbooks, solutions manuals (and A2L readings for blind and visually impaired students) and sign your e-text agreement.

Textbooks can be purchased from any bookstore or online retailer (e.g., the Campus Store or Amazon) and can be bought used. However, we cannot accept receipts for books purchased from other students or textbook rentals. If you have already purchased a textbook from a classmate, please get in touch with the LAS Coordinator for alternatives. You can find a link to the textbook listings here.

Students wishing to meet with the LAS Coordinator can click here to book an appointment.

Please note: It can take up to 4-6 weeks to receive files from publishers (average is 1-2 weeks), particularly during busy periods. Please know that we will work on your request as soon as we receive it, but we will only contact you if there is a question once your files are ready to share. Please request your material as soon as possible. Even if you only know the names of your courses, we can help with the rest.

Please allow approximately 10 minutes to complete this form.

Please feel free to reach out to liblas@mcmaster.ca if you have any questions!
Please note, we will do our best to obtain and produce it in the format requested, however, it may not be available.
What semester are these textbooks for? (i.e. Winter 2025)
Please request texts one semester at a time.
Please include course code, section, and instructor's name.
E.g. Psychology 1X03 C01, Dr. Freud.
Please list all of the textbooks and ISBNs (if known) for the course that you plan on purchasing. You don't need to have purchased the text already, but a receipt must be provided for the files to be shared with you.
Would you like to make an alternate format request for a second course?
If you are requesting our assistance with class notes, please contact us. We only have the capacity to convert materials for blind and visually impaired students; however, we can assist your instructor in adding the SensusAccess program to A2L so you can download alternate formats in Avenue to Learn.
Statement of Agreement
By clicking on "I agree" below, you agree to the following terms:

A request has been placed for alternate format material for courses that I am taking this term. These texts are required material for the courses in which I am currently enrolled. I agree not to make copies and/or further distribute the electronic texts received. I understand that copying and/or distributing the file violates both university policy and the university's agreement with publishing groups and/or electronic text producers. I further understand that copying and/or distributing these electronic texts may jeopardize my future access to this material through McMaster University. I agree to delete these files upon completion of these courses, or when I no longer own the text.
